A Plymouth man has died after authorities said he suffered two heroin overdoses in less than 12 hours.

The victim, 43, was found by his mother during an overdose around 8:30 p.m. on Thursday at his apartment, the Boston Herald reported. After authorities arrived and administered Narcan to combat the overdose, he was taken to Beth Israel Deaconess Hospital.

He checked himself out of the hospital on Friday morning and was found again by his mother having an overdose around 8:30 a.m., the Herald stated. He was taken to the hospital where he was pronounced dead.

At the time of his second overdose, police were headed to his home as part of a program where authorities check on patients within a day of an overdose, WCVB stated, but they arrived too late.

“We’ve had some people get Narcanned two or three times in a couple of days. But I can’t think of one where we had them overdose, he was Narcanned, left the hospital and overdosed that quickly. That seems to be unprecedented,” Plymouth Police Chief Michael Botieri was quoted by the Herald.
